Submitting its report to Minister for Culture, Tourism and Civil Aviation Kripasur Sherpa, the commission said the helicopter subsequently became uncontrolled and crashed on the sloppy mountain terrain after breakdown of its dynamic gears.In addition to that, the commission has outlined passenger distraction, lack of situational awareness, cumulative fatigue, personality traits, lack of wire avoidance trainings, and no obstacle markings as contributing factors for the accident.
The commission has given safety recommendations to Civil Aviation Authority of Nepal (CAAN), Mountain Air and Nepal Electricity Authority for improving aviation safety.
The investigation commission was led by Buddhi Sagar Lamicchane, joint secretary of tourism ministry. Similarly, Captain Sidhhartha Jung Gurung and Engineer Janak Thapa were members in the commission and Pramod Nepal, under secretary at the tourism ministry, was the member- secretary.
